1. Referring physician name and a written referral if required by your insurance (ALL patients need a referring physician)
   
2. Insurance information
   
3. Reason for consultation
   
4. Physician requested

For your clinical appointment, please bring:

1. All recent medical records and laboratory studies
2. All radiographic images and their reports
3. A list of all medications or the actual medication vials
4. Names of previous physicians seen for the current neurological issue
5. In order to simplify your initial visit please download and complete a registration form if possible

It is the patient's responsibility to bring in all records and proper information to maximize clinical care. If proper information is not brought to the visit, this may delay the clinic appointment, diagnosis, treatment, or duplicate testing.
