Dr. Adam J. Wolff most recently hails from the prestigious Stanford University where he completed a fellowship in Neurophysiology. He is a Chicago native who graduated from the Rush Medical College and completed his neurology residency at Northwestern University. He is board certified in Neurology and Neurophysiology and serves as the Director of Neurology and Stroke at Porter Adventist Hospital. Dr. Wolff welcomes patients with all neurological problems and has special interests in EMG and Botox therapy.
Dr. Justin S. Moon most recently comes from the world-renown Mayo Clinic in Rochester, Minnesota where he completed an accredited fellowship in Headache and Facial Pain and is a certified expert in head pain. He is a native of Phoenix, Arizona and completed a Masters in Public Health and his Doctorate of Medicine at the University of Arizona. He completed his internship and residency at the Medical College of Wisconsin (Marquette School of Medicine). He is board certified in Neurology and sees general neurology patients and subspecialty referrals for headache and facial pain. Dr. Moon offers Botox therapy and peripheral nerve blocks for pain. Dr. Moon is a member of Denver Medical Society's Board of Directors and is a consultant physician to professional sports teams and University of Denver NCAA teams.
Dr. Susan P. Anzalone most recently comes from University of California at San Francisco (UCSF) where she completed a clinical fellowship in Multiple Sclerosis (MS) and Neuroimmunology. She grew up in Colorado and completed her undergraduate studies at the University of Colorado at Boulder before going on to medical school and neurology residency at Loyola University Chicago Stritch School of Medicine. Dr. Anzalone has provided medical care to patients in Ecuador, Kenya, and Uganda. She has a holistic approach aiming to treat not just the disease but the whole person, with special emphasis on diet, fitness, stress management and overall wellness. Dr. Anzalone is a board certified neurologist who has a special interest in autoimmune diseases such as MS and Neuromyelitis Optica (NMO) but welcomes all general neurology patients.

Jordan Mast is a graduate of Midwestern University in Chicago. He is from Bloomington, IL and attended Weslyan University. A board certified physician assistant with an emphasis in general neurology, he has a special interest in patient education and offers multiple services including Botox therapy and peripheral nerve blocks for pain.
Stefanie Eymar, MSN, FNP is an ANCC board certified family nurse practitioner. She is from Kansas City but has enjoyed the great outdoors of Colorado for the past 13 years. After acquiring seven years of experience as a registered nurse, she obtained a Master’s degree from Regis University in Denver, Colorado in 2014. Her career includes four years of critical care experience in the field of neurological/trauma, garnering her increased expertise in traumatic brain injury and stroke care. She provides patient centered care while treating neurological diagnoses. She also provides Botox for migraine therapy.
