- NO DISABILITY PAPERWORK.
- We will not refill medications in the evenings or weekends.
- Evening and weekend phone calls are for emergencies. Headaches and non-emergency issues will be handled durning regular business hours.
- All cancellations must be notified 24 hours in advance otherwise a $50 charged will be assessed.
- NO SHOW fee is $50 and must be paid prior to next scheduled visit.
- Patients with more than 2 no shows will be discharged from the clinic.
- New patients should arrive 15 minutes early for their appointment in order to complete paperwork. You can download the New Patient forms ahead of time and is highly suggested! If a patient is more than ten minutes late for an appointment the indidual may be asked to
reschedule.
